The Collaborative Cultural Competence (C3) in Advance Care planning is a systems approach framework to tackle issues with low Advance Care Planning and Advance Directive completion in American adults and especially African Americans. The factors accounting for the continuous gap in AD completion among African Americans are not well understood. This book initiates the Advance Care Planning Conversation via a quality improvement project in a faith-based community. Collaboration at the micro and macro systems level is important between health care professionals in the acute and primary care settings, faith leaders, and health care providers in their communities. This approach allows patients to receive the care they need while in the community, rather than in an admitted hospital bed.

The outcome is a gift of clarity for the patient, their family, and the health care professionals – a win-win.

Villeroy Tah was born in Bamenda-Cameroon. Together with her husband, they have four children. Villeroy started her career in the healthcare industry as a Certified Nurse Aide to a Licensed Practical Nurse (LPN) and a Registered Nurse. She attended MassBay Community College, Salem State College (BSN), Worcester State University (MSN), and Elms College (DNP). Villeroy's quest for knowledge, skills and better patient care has been a driving force for advancements in her career from being a staff Nurse, to beciming a Supervisor, Manager, and  Director of Nursing services. Villeroy worked with Department of Public Health as a Surveyor for Health Care Facilities across the Common Wealth of Massachusetts. As a Faculty staff, Villeroy is passionate about educating novice nurses in ways that impact the nursing practice as a whole. Villeroy is a devoted Public Health Nurse who engages in advocacy work that seeks patient safety, health equity, and justice for all accessing the healthcare system.
