Team Colorado's Hotel2Hospital (H2H) Playbook describes the "just-in-time" conversion of a mid-size hotel into an Alternate Care Facility (ACF) capable of hospital-level care as a response to a large medical surge event. The H2H Playbook outlines how to convert any hotel with a minimum of 150 rooms into an ACF in 2-4 weeks and details considerations for operating the ACF as a "wing" of a hospital for 3-4 months. To begin, the H2H Playbook outlines the rationale for using a hotel as an ACF and hotel selection criteria for two hotel types: a mid-size hotel and large conference hotel. After a hotel is selected, the H2H Playbook recommends strategies to manage the hotel conversion within a hospital's existing incident management framework and determine how to use, augment, and add to existing hotel infrastructure to create a hospital-like care setting. The H2H Playbook provides: an approach for converting hotel rooms into patient care rooms for both intensive care and medical/surgical patients; a list of clinical and non-clinical equipment; an estimation of staff numbers and types; considerations for how to establish clinical services such as Lab, Pharmacy, and Radiology; and suggestions for providing support services such as food services, laundry, environmental services, and security in a hotel. Additionally, the H2H Playbook includes architectural drawings, engineering plans, checklists, and other resources to support hotel conversion. Finally, the H2H Playbook offers recommendations for operating the H2H as a "wing" of the Partner Hospital to include patient admission, bed assignment, and expanding existing hospital services and the electronic medical record to include the H2H. In addition to the H2H Playbook, Team Colorado also created an H2H Estimator Tool that estimates H2H patient beds, staffing needs, equipment costs, and construction costs by inputting certain characteristics; an H2H Conversion Cost Sheet with estimated conversion costs for both hotel types; and an Emergency Response Plan Template that can be customized for the H2H.

The Hotel2Hospital project was funded by the Department of Defense as a potential solution to its medical surge planning scenario of 1,000 combat casualties per day for 100 days returning to the United States from an overseas conflict. Charles Little, DO, FACEP (an emergency physician and Medical Director of Emergency Preparedness at the University of Colorado Hospital) and Jason Persoff, MD, SFHM (a hospitalist and Assistant Medical Director of Emergency Preparedness at the University of Colorado Hospital) gathered a group of clinical personnel as well as experts in project management, hospital and hotel architecture, and hospital operations to develop the H2H concept. Team Colorado includes representatives from the University of Colorado Denver, All Clear Emergency Management Group, UCHealth, Davis Partnership Architects, Cator Ruma and Associates, and Martin & Martin Consulting Engineers. Most of the individuals on the project team had firsthand experience designing, building, and operationalizing Alternate Care Facilities (ACF) in Colorado during the COVID-19 pandemic. They infused their knowledge of emergency response and experience with COVID-19 into this Hotel2Hospital project. The result of this collective effort is this Hotel2Hospital Conversion Playbook.
