Team Colorado's 2025 Hotel2Hospital (H2H) Conversion Guidebook describes the "just-in-time" conversion of a suitable hotel into an Alternate Care Facility (ACF) capable of hospital-level care within 2-4 weeks as a response to a large medical surge event. To begin, the H2H Guidebook outlines the rationale and selection criteria for two hotel types: a mid-size hotel and large conference hotel. After a hotel is selected, the H2H Guidebook describes how to convert hotel areas and rooms to clinical and support spaces and how to use, augment, and add to existing hotel infrastructure to create a hospital-like care setting. The H2H Guidebook includes architectural drawings, engineering plans, checklists, proven practices, and other resources to support hotel conversion. Finally, the H2H Playbook offers recommendations for operating the H2H as a "wing" of a Managing Hospital to include patient admission, bed assignment, and expanding existing hospital services and the electronic medical record to include the H2H. In addition to the H2H Guidebook, Team Colorado also created an H2H Estimator Tool that estimates H2H patient beds, staffing needs, equipment costs, and construction costs by inputting certain characteristics; an H2H Conversion Cost Sheet with estimated conversion costs for both hotel types; and an Emergency Response Plan Template that can be customized for the H2H.

About The Author


Charles Little, DO, FACEP is an emergency physician and Medical Director of Emergency Preparedness at the University of Colorado Hospital. He is a Professor of Emergency Medicine and Public Health at the University of Colorado Denver and the Colorado School of Public Health. Dr. Little served with the NDMS Disaster Medical Assistance Team (DMAT) CO-2, is a physician advisor for the North Central Region Healthcare Coalition, and advised CDPHE on Crisis Standards of Care and Medical Operations. He was CMO for the Colorado Convention Center Alternate Care Facility. He is Co-PI and Medical Director of the HHS/ASPR funded Mountain Plains Regional Disaster Health Response System for Federal Region VIII.
